Why isn't my checkbox appearing in my Google form
If you click on 'Sign the Pledge' and then advance to the second page, you'll see the last field should be a checkbox - it's set up in Google Docs as a checkbox yet for some reason appears as a text field.
Anyone know what i'm doing wrong?
FYI the problem no longer appears in the Sprout - I found a workaround using radio buttons... for some reason they work.

Yes, we had some problems with check boxes a few weeks ago, but both checkboxes and radio buttons should be working fine now.
