That's a great Sprout Robert! Very interactive with cool images.

You can create a mail form or submission form like Robert's using the Google Forms service under the components panel of the Sprout Builder. All you need to do is create a Google spreadsheet using Google Docs (you must have a Google account) and then add the URL of the spreadsheet to your Sprout. Then, whenever someone fills out the form, his or her answers will be updated to your spreadsheet in your Google Docs account (you can find more info by searching Google Forms in the Help tab).

I wish there was a notification when there are replies. I have a few accounts and sometimes forget to check the forums. But yup google docs works well for that as well as feedback forms. I just added one in a few minutes to my www.robertricks.com website and it solved a problem I was facing. Additionally these can help with web developers who may not have scripting background to create feedback forms for clients.

Sorry took so long Jeff...
The purpose of the card is to allow users to show off their MMO characters and what they are interested in. Also would like to be able to let them know when they have missions, etc. It's part of a larger thing I'm trying to get off the ground. SPROUT allowed me to rapidly prototype the card to show partners what i am proposing...
